Community Health Workers Since 2008, HRHN has been at the forefront integrating Community Health Workers (CHWs) with chronic disease management services. Studies have shown that CHWs can help patients avoid costly hospitalization and emergency department visits, in addition to helping providers and hospitals by decreasing unnecessary visits and uncompensated care.
